Not true. You can create (or borrow) higher resolution assets and scale them down to fit the size a game uses. Some hacks, especially complete hacks that add new levels and characters, that use different size assets compared to the original definitely need to take mistake 2 into account.

For #2 for romhacking, I would say using assets from clashing art styles. IE - don't mix sprites from SMW and Yoshi's Island.

Also for romhacking, I would add "not paying attention to palettes". I don't know how many FF6 hacks put straight white (#FFFFFF) into character sprites, when the overall game palette never uses that color.

They talk much about
don't use pure black borders with sprites in indie games because the LCD saturation now.

Some games has very line-cut tiles, as Ff5 from Steam.
The meteor tiles, with lines between tiles.
Even in world map.
Giana sisters hd too in some stages. Fails directly.
