Did you read the readme? It seems like you need access to the game's filesystem, and just replace the old files with the new ones in the archive.

Yes, I read it:
"Use files included in this package and overwrite the existing original ones on a disc"
So what program do I use to perform this action?

I've never done it myself so I don't know for sure, but I believe you'd need a modded xbox that lets you install/run games from the internal hard drive. I assume you'd copy the files to that hard drive. I don't think the particulars of how to do that are allowed to be discussed here, but I'm sure you can find more info elsewhere.

If you want to burn the translation to a disc, you'll need a program to rebuild the ISO.

I already have a soft modded Xbox that burns burned games. I ripped my ISO of Togue R, what program do I use to rebuild said ISO with this patch? Very vague instructions in the readme.

I've used Qwix for XBox ISO Operations in the past. I don't recall if I ever used it to rebuild from files to ISO though (it does have a create ISO option), because usually I just transfer the files directly to the XBox HDD and play from there. I can't even play from disc these days, as the drive tray won't open anymore. From what I've read, this is a common thing. If for some reason you could only play the translation from disc, that would actually be bad for that reason.

I mean, how do you even do that? I believe I could boot the game from my hard drive, but it would take up half my HD space so that's why I only play off disc. How would you even get a working patched version of the game running off the HD? Don't you need to patch the ISO anyhow?

The Xbox HDD is replaceable with standard PC drives, most modded I've seen on Ebay back when I got mine already had considerable larger ones, the one I got has 80GB.

For playing from there, you can transfer the extracted files to a folder (maybe this depends on the mod), then start the executable. I think some games might need a hdd fix or something when you directly transfer from original DVD, I usually got mine from the web and never had to patch them. I can't transfer my originals directly because of the broken drive now anyway, so I use the FTP method.

hdd fixes? hmm... I don't know what games have issues when installed.

Yeah replacing the hard drive is not required to install games. Except it's futile with under 8GB usable space.
